I had physiological implants installed in 1998. They are placed through the armpit and under the muscle. I started noticing wrinkling on my left breast. I would like to replace them with silicones and was wondering if it is possible to do the replacement using the same technique to avoid breast scarring?

Implant replacement is completely possible using the same technique through the armpit, which allows avoiding breast scars. This method is called "transaxillary" and is very popular because it leaves no visible scars on the breasts. Also, if your previous implants were placed under the muscle, you can keep it during the replacement, as this position provides a more natural look and a lower risk of complications.
